Once you click "OK" and the effect is applied to a new layer, add a Layer Mask to the layer (Click the rectangle with a circle icon in the Layers panel).
Press Ctrl+I (Windows) or Cmd+I (Mac) to Invert the mask (it turns black). The smoothing effect disappears.
Select the Brush Tool (B).
Make the Foreground color White.
Paint over the skin areas only.
Result: The smoothing is now restricted only to the skin, leaving eyes, teeth, and hair sharp.
Small: Targets fine pores.
Medium: Targets slight unevenness.
Large: Targets larger patches of uneven tone.

Use Portraiture on a separate layer or virtual copy
Refine mask around eyes, hair, lips, and jewelry to prevent smoothing
Combine with localized frequency separation for texture work when required
