Definition and Prevalence
- Varicocele is essentially a swelling of the veins that drain the testicle. It's a condition more commonly diagnosed in adolescents and adults but can be identified in prepubertal boys as well.
- Prevalence: Varicoceles are found in about 15% of boys and adolescents, usually becoming apparent during puberty.
- Testicular hypotrophy (volume difference >2 mL or >20% by orchidometer).
- Pain or discomfort (rare in children).
- Bilateral varicocele (prophylactic surgery).

- Also widely used. It involved ligation of the internal spermatic vein and artery in the retroperitoneal space (higher up than Ivanissevich).
- Advantage: Technically easier and lower recurrence rate because the artery and vein are ligated together in a bundle.
- Disadvantage: Ligation of the testicular artery was controversial; however, in 1982, surgeons relied on the collateral arterial supply (vasal and cremasteric arteries) to keep the testis viable.
